AHS Rural Palliative Care Consultation Team 
(Call PCN or Homecare for these)
  • A health care professional can initiate a referral by phone or fax.
  • The most appropriate and available Consult Team member will respond to the consult request by connecting with the patient, family and /or local health care provider(s).  Other consult team members will get involved and more visits will be made later as needed.
  • Most meetings with the Consult Team occur in person or over the telephone.  Sometimes a meeting takes place by videoconference.
